Total Rewards Employee Benefits Package
The following is the list of employee benefits currently available at the Regional Food Bank of Oklahoma as of January 1, 2010. Employee eligibility varies and is addressed separately with each program.
Medical Coverage (Community Care)
Employees have two different health programs to choose from, which are provided by Community Care. You must work at least 30 hours per week to be eligible.
The choices include: an HMO or PPO. The Food Bank covers 95% of the employee's monthly premium and 45% of employee spouse and dependants.
Prescription Drugs
No matter which Community Care health program chosen, the employee will get prescription drug benefits. Express Scripts are available in all major retailers as they are the nation's largest prescription drug provider. The plan has three levels of co-pays:

A formulary is a list of preferred prescription drugs that are covered by a benefit plan. A formulary list is available at http://www.ccok.com.
Life Insurance, Accidental Death & Dismemberment
The life insurance and AD&D programs are provided through Guardian Life Insurance Company. To be eligible, the employee must regularly work at least 30 hours a week. Voluntary life insurance is available for the entire family. The Food Bank provides employees with $10,000 of life insurance and basic short term disability coverage, free of charge. The employee has the option to purchase additional coverage if desired.
Flexible Spending Account (FSA)
A flexible spending account, sometimes referred to as a cafeteria plan, is available to all Food Bank employees. A FSA has two parts: medical and child care & dependent care assistance. The employee can put in up to $5,000 for medical and related expenses; and up to $5,000 for child care and dependent care expenses. The money is taken out in equal amounts from each of the employee's pay-checks during the year. An FSA offers two benefits: amounts designated reduce the employee's taxable income and eligible expenses are purchased/reimbursed tax free. A Benny debit MasterCard Card is provided, so the employee has no out of pocket expense. A Benny card is just like a credit card except the money spent on the card is the total money amount the employee elected for FSA participation.
Pre-Paid Legal Services
All employees are eligible regardless of the amount of hours worked per week. Legal services included in the program: legal consultation and advice, letters and phone calls made on behalf of client, contract and document review, will preparation, motor vehicle legal services, up to 60 hours of attorney time for trial defense services, and IRS audit legal services. The employee is responsible for 100% of the legal services monthly premium.
Vision Benefits
All employees working at least 30 hours per week are eligible for vision coverage through VPS. The VPS has a network of over 33,000 optometrists, ophthalmologists and retail vision care centers in the U.S. The employee may also use providers outside of the network, but the benefit will be less. Benefits for exams and lenses can be used once every 12 months and for frames every 24 months. No ID card is needed, simply make the appointment, tell them you are a VPS member and give them your social security number and date of birth. The employee is responsible for 100% of the vision benefits monthly premium.
Dental Benefits
All Food Bank employees working at least 30 hours per week are eligible for dental coverage. The dental coverage is through Guardian and the employee has a choice of two networks – a Base Plan or a Buy-Up Plan, wherein benefits are increased. The employee is responsible for 100% of the dental benefits monthly premium. For a list of providers in the networks, visit http://www.glic.com.
Guardian Work-LifeMatters Program
When you need help with non-acute emotional, mental or behavioral problems such as depression, anxiety, relationships or alcohol and drug problems, a Work-LifeMatters consultant will help access the situation and provide resolution consulations or may authorize up to three office visits with a counselor in your area. The consultant may also assist you in obtaining behavioral health treatment, if needed, through your health plan.
401(K) Retirement Plan
All employees are eligible after one full year of employment with the Food Bank for the 401(K) Retirement Plan administered by ADP. The Food Bank will match the first 4% of the employee's contribution to their retirement. Employees are eligible on the first of the quarter following one year of service.
Personal Time Off (PTO)
Paid Time Off ("PTO") will accrue biweekly beginning the first day of employment based on hours worked and is to be used for vacation, sick and medical leave. Eligibility for PTO in any one year shall be based upon length of continuous service. PTO pay shall be based upon the employee’s wage rate.
Beginning on the first day of employment, an employee shall begin earning PTO, although no PTO will be granted to any employee until he or she has completed the 90-day orientation period. Regular employees will earn annual leave based on hours worked and tenure as follows:
Employees hired after July 1, 2010:
- 0-5 Years of Employment .06923 per hour worked
(Equates to 18 days per year based on full-time status of 40 hours per week.)
- 5-10 Years of Employment .07692 per hour worked
(Equates to 20 days per year based on full-time status of 40 hours per week.)
- 10+ Years of Employment .0847 per hour worked
(Equates to 22 days per year based on full-time status of 40 hours per week.)
Employees hired prior to July 1, 2010:
- 0-5 Years of Employment .0693 per hour worked
(Equates to 18 days per year based on full-time status of 40 hours per week.)
- 5+ Years of Employment .0847 per hour worked
(Equates to 22 days per year based on full-time status of 40 hours per week.)
Part-time employees working 20 or more hours each week are eligible for PTO using the accrual rates listed above.
Major Medical or Emergency Leave
An employee will be able to designate a portion (not to exceed 72 hours if employed up to five years or 88 hours if employed five years or more) of his/her PTO to an Extended Medical Leave Account that could be used for major medical or emergency leave. Any leave designated in this category will not be able to be used for anything other than major medical or emergencies and cannot be moved back to the employee’s PTO. Examples include, but are not limited to, birth of a child, surgery, car accident (requires hospitalization or doctor certificate stating you are unable to work due to the injury), unexpected hospitalization for the employee or an immediate family member and/or death of an immediate family member. The employee will not be paid for any accumulated extended medical leave upon termination or voluntary resignation.
Effective June 1, 2010, employees are able to accumulate the totals below in their Major Medical/Emergency Leave Accounts based on tenure:

< 5 years 240 hours
5-10 years 480 hours
> 10 years 640 hours
Holiday Pay
An employee eligible for holiday pay shall receive pay based upon the number of hours regularly scheduled at the employee’s wage rate for the following holidays:
- Floating Holiday
- New Year’s Eve Day
- New Year’s Day
- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving Day
- Day following Thanksgiving Day
- Christmas Eve Day
- Christmas Day
The holiday schedule will be provided at the beginning of each calendar year. If any of the above named holidays falls on a Sunday, the leave will be granted on Monday. In the event the holiday falls on a Saturday, the Executive Director will determine whether the leave will be granted on Friday or Monday.
Effective June 1, 2010, employees must have completed six months of employment, before they are eligible for the Floating Holiday.
Part-time employees are eligible for holiday pay based on the number of hours regularly scheduled, if the holiday occurs on a regularly scheduled work day.
